Ontario’s primary care sector remains at a critical juncture - the healthcare system is continuing to undergo significant transformation, creating both instability as well as opportunity.

AFHTO’s 2024-2027 strategic plan was developed during a time of significant change, both within the organization and across the broader healthcare system.

Representing the input of our diverse membership, staff, partners, and health system leaders, AFHTO’s 2024-2027 Strategic Plan sets the priorities that guide our work, from strengthening system partnerships to supporting high-performing interprofessional teams. We are excited to work with our members and partners to put our strategic plan into action.

Our North Star Report

As a proud leader of primary care teams across Ontario, we present our bold vision for creating high performing teams that provide compassionate, comprehensive care, ensuring timely and equitable access for every Ontarian.

 “Our North Star: Creating a Future-Focused Primary Care System Together” (October 2025) highlights the vital role of primary care teams in delivering high-quality, coordinated, and equitable care to 25% of Ontarians and counting. It details current challenges, such as workforce shortages, funding gaps, and system inefficiencies, and presents AFHTO’s commitments to improve care access, digital integration, governance, and collaboration. The report calls for strong partnerships with government to achieve universal attachment to primary care by 2029, improve health outcomes, and build a sustainable, high-performing system for all Ontarians.
