To show the full reach and impact of team-based primary care, this map includes all government-funded primary care teams across Ontario. The information presented is drawn from publicly available sources and is intended to help patients, health system partners, and community stakeholders identify and connect with primary care teams in their communities.

Recognized primary care team models (due to funding or operating model): 

• FHT – Family Health Team
• IFHT – Indigenous Family Health Team
• NPLC – Nurse Practitioner-Led Clinic
• CHC – Community Health Centre
• AHAC – Aboriginal Health Access Centre
• IIPCT – Indigenous Interprofessional Primary Care Team
