WHO WE ARE

AFHTO is a non-profit, member-based organization dedicated to strengthening team-based primary care across Ontario. Membership is voluntary for new and existing interprofessional primary care organizations, including Family Health Teams, Nurse Practitioner–Led Clinics, Community Health Centres, and other interprofessional primary care organizations.

Lead health system and primary care transformation and be an essential partner for change.

AFHTO continues to be a leading voice in the sector and with government as our health system undergoes transformation, particularly as the maturation of Ontario Health Teams and Primary Care Networks continues. Through regular communication, AFHTO supports and equips its members with the tools to engage in grassroots advocacy on key issues in their communities.

The AFHTO office in Toronto is located on the traditional territory of the Mississaugas of the Credit, the Anishinaabeg, the Chippewa, the Haudenosaunee, and the Wendat peoples, and is now home to many First Nations, Inuit, and Métis communities.

Because our members serve communities across the province, we also recognize that AFHTO’s work takes place on the traditional lands of Indigenous Nations throughout Ontario – including territories governed by longstanding Treaties, the Williams Treaties, and lands where Treaties remain unresolved.

We honour the ongoing presence, rights, and stewardship of Indigenous Peoples and acknowledge the responsibilities this brings to our work. As an organization focused on improving primary care, we are committed to advancing approaches that support health equity, uphold the spirit and principles of reconciliation, and contribute to better health outcomes for First Nations, Inuit, and Métis communities across Ontario.
