1. What is AFHTO?
The Association of Family Health Teams of Ontario (AFHTO) is a not-for-profit member-based organization representing primary care organizations and their teams in Ontario. Our members include Family Health Teams, Nurse Practitioner-Led Clinics, Community Health Centres, and other interprofessional primary care models. We work to advance high-quality, equitable, and comprehensive care for Ontarians by supporting and advocating for the people and teams delivering that care. AFHTO does not govern its members.
2. What is “team-based primary care” and why does it matter?
Team-based primary care brings together a range of health professionals – such as family physicians, nurse practitioners, nurses, social workers, pharmacists, dieticians, physiotherapists, and more – to deliver coordinated, patient-centered care. This approach ensures patients receive the right care from the right provider at the right time. Research shows that team-based care leads to better health outcomes, higher patient and provider satisfaction, and more efficient use of health system resources.
3. Who can become a member of AFHTO?
Membership is open to interprofessional primary care organizations in Ontario, including but not limited to Family Health Teams, Community Health Centres, and Nurse Practitioner-Led Clinics. 4. What are the benefits of AFHTO membership?
Members gain access to a strong community of peers, exclusive resources, advocacy support, and data-driven tools that help strengthen their work. Membership also includes member-only webinars, working groups, the annual AFHTO Conference, and opportunities to share innovations and influence provincial policy. As well, members gain access to a variety of group benefits such as eligibility for HOOPP and the Community Health Ontario Group Insurance Plan, discounts to several organizations, and access to job posting on AFHTO’s primary care job board at no charge.
AFHTO also amplifies members’ voices in discussions with Ontario Health, the Ministry of Health, and other key stakeholders.
5. How can my team or organization join AFHTO?
Visit our “Become A Member” page to take a look at eligibility criteria and complete the online application form. Once you’re approve and membership fees are paid, your team will receive onboarding support and access to member benefits. If you have questions or would like to discuss membership before applying, we’d be delighted to provide more information.
6. How does AFHTO advocate for primary care in Ontario?
AFHTO works to ensure that team-based primary care is recognized, supported, and strengthened within Ontario’s health system. We advocate for sustainable funding, equitable access, and policies that enable interprofessional teams to deliver comprehensive care. Through partnerships with Ontario Health, government, and other associations, we share data, evidence, and success stories that highlight the value of primary care teams to the broader system.
7. Does AFHTO offer events or conferences?
Yes! AFHTO hosts an annual conference that brings together leaders, interprofessional healthcare teams, administrators, policymakers and more from across the country. The event features keynote speakers, workshops, and the Bright Lights Awards, which showcase innovations in primary care. Members and non-members are welcome to attend. 8. How can I stay informed about AFHTO’s work and updates?
You can stay connected by subscribing to our e-newsletter and following us on social media (LinkedIn, X, and Instagram). We share regular updates on primary care policy, advocacy, events, and member success stories. Members also receive targeted communications with practice resources, data reports, and opportunities for engagement!
9. How does AFHTO collaborate with partners and stakeholders?
